Chesterholme

Chesterholme, an assessment and intervention unit, is itself a perfect example of helping people along their own particular care pathway. It provides an environment where someone can progress through the main service to the step-down Graded Rehabilitation Annexe, on the same site, and where they learn about and experience more independent living.

It is a place to get the basics back in place and carry out specialist assessment, intervention and planning for the future.

The main part of Chesterholme provides 24 individualised places for adults with learning disabilities and behaviours that challenge, including those with complex needs and dual diagnoses. Here they are encouraged to live their own life to their optimum potential, accessing the community with support, and enjoying a range of educational and recreational activities as well as receiving 24-hour nursing support.

Chesterholme’s “step down” Graded Rehabilitation Annexe has five places and is a service where people can learn life skills, access the community more independently and do more and more for themselves – from meal planning to general household tasks to organisng finances. This is a positive place – full of enthusiasm and encouragement.

Chesterholme (along with Oaklands) is also home to the Right Sew and Sews – an ambitious group of residents who have formed themselves into a social enterprise business, selling their crafting works and expertise.

Working to commission, and also to produce pieces for sale at local (self-organised) fetes and fairs, this group is inspiring and hugely talented. Click here to read more about the Sew and Sews.
